QUALIFICATIONS

Must be 18 years of age or older

A fitting evaluation must be completed by an appropriate medical provider (physical therapist or occupational therapist)

You must have exhausted traditional funding options such as Medicare, Medicaid, or private Insurance. Or be unable to obtain coverage.
Ready to Apply
Required Documents Include
- Seating Evaluation from your medical provider
- Insurance denial or explanation of ineligibility for coverage
- Letter of Medical Necessity from your provider
